Phenazopyridine hydrochloride tablets, 100 or 200 mg, 100-count bottles. Manufactured for: Burel Pharmaceuticals Inc. or Nationwide Laboratories LLC.

Salsalate tablets, 500 or 750 mg, 500-, 100- and 1,00s0-count bottles. Manufactured for: Nationwide Laboratories LLC.

Metformin hydrochloride tablets, 500 mg, 1000 tablets. Manufactured by: Alkem Laboratories Ltd.
